Melasma Treatment by a Dermatologist: Synergistic  use of Topicals and Safe Lasers

A multimodality approach is often needed to manage facial pigmentation concerns.

Melasma is a chronic pigmentation disorder that requires initial resolution and ongoing maintenance therapy to prevent a recurrence. Although it is not curable, effective treatment involves a synergistic combination of tinted sunscreens, lasers, peels, and tablets.

Causes of melasma are multifactorial, including genetic and hormonal factors, excess UV and visible light exposure, and excessive friction/heat. Effective treatment involves treating both the superficial and deep pigment elements.

Photoacoutsitic Laser treatment targets deep pigment and gradually breaks it up, while prescriptive tuitional and rotational topical creams treat the superficial pigment. A synergistic combination of treatments leads to effective results, but treating melasma takes time. It may take up to 3- 6 months or longer for initial fading, followed by ongoing maintenance to prevent recurrence.

Freckles, moles, old age spots, solar lentigos (freckle-like lesions induced by the sun), chronological and photo-ageing are all causes of irregular hyperpigmentation. Before cosmetic treatment of this problem is undertaken, it is essential to first seek the advice of a dermatologist who can exclude any sinister problems (eg abnormal mole) prior to cosmetic treatment.

How is pigmentation treated?

This depends on the type of pigmentation that you have. There are many different types of pigment reduction treatments available which include:

  • Topical bleaching agents (eg Kojic acid, tretinoin or azelaic acid)

  • Chemical bleaching (eg lactic acid chemical peels)

  • Laser pigment reduction

  • Combination treatment: often a combination approach is used depending on the type of pigmentation being treated
